    Dental Council Statement - to be completed

    Dental Nurses qualified within Ireland;

    1. Complete application form
    2. Current Curriculum Vitae or signed work history (This must be a detailed account of your work history from the time you graduated in Dental Nursing but should include any dental nursing experience gained before you qualified. Dates must be formatted as DD/MM/YYYY and you must state the title or grade of your post. Your work history must include full name and address of each employer.
    3. Remittance of €15. To pay online click the "Pay Now" button and follow the instructions. The fee payable is listed under "Application fees". Be sure to select "Dental Nurse Application Fee" to avoid unnecessary refunds and print a copy of your receipt to attach to your application. We will also accept a personal cheque as an alternative method of payment.
    4. Photocopy of your Certificate in Dental Nursing
    5. Photocopy of your passport for identification purposes
    6. Complete Dental Council statement (This statement must be completed. If you have registered in any country as a dental professional, you must declare this information on your statement. If you have never registered anywhere else before, simply sign and date the statement.)

    Dental Nurses qualified from the UK;

    1. Complete application form
    2.     6. Letters of Good Standing (You must submit original Letters of Good Standing from any regulatory body/ Competent Authority that you are currently, or have previously been, registered with)
    7. Complete Dental Council statement (This statement must be completed. If you have registered in any country as a dental professional, you must declare this information on your statement. If you have never registered anywhere else before, simply sign and date the statement.)

    Dental Nurses qualified outside of Ireland and the UK;

    Please note that any Dental Nurse qualified outside of the UK and Ireland must submit an application for assessment for the purposes of registering. Below is the information that must be submitted for assessment. After assessing your application the Council may require you to submit further information.

    2. Photocopy of your passport for identification purposes

    6. Transcripts of Course in Dental Nursing – You must submit the originals and these should include subjects undertaken, grades received and any clinical training
    7. Course Syllabus (This must be an official syllabus, obtained from the awarding university and must contain a detailed outline of what your Diploma in Dental Nursing entailed. The original version must be submitted. A photocopy will not suffice.)
    8. Scope of Practice (If you have practised a regulated profession in a member state, please include the scope of practice you worked under)
    9. Completed Dental Council Statement (This statement must be completed. If you have registered in any country as a dental professional, you must declare this information on your statement. If you have never registered anywhere else before, simply sign and date the statement.)
    10. Letters of Good Standing (You must submit original Letters of Good Standing from any regulatory body/ Competent Authority that you are currently, or have previously been, registered with)

     Important information regarding the translating of documents;

    Any documentation submitted, if in a language other than in English, should be translated into the English language and duly notarised. Both the original requested document and its original translation should be forwarded.Notarised translations must be an exact translation of the source material. They must always include a clause from the translator, together with the translator's seal affixed and must be certified by the translator's signature. A copy of the document that has been translated must be bound to its notarised translation by the translator, making it clear that he/she has made the translation from the specific document in question.
